The development and creation of Ambulatory Interdisciplinary Clinical Services that provide support to patients in a convenient and low barrier location is paramount to patient and clinician health and satisfaction. This project was aimed at an evolution of service delivery for patients to support both acute care capacity and improve patient outcomes.

This project, while challenged by many temporal obstacles and logistical barriers, has served as a pilot to demonstrate what can be achieved when a true partnership is created between Clinicians and Administration. While not yet perfect in outcome and deliverables, it produced an exercise to assist the development of a more efficient project map in the future.
